The inquiry form on this website is not connected to a lead system and does not transmit or store information. This notice must be reviewed and updated to match Cardinal Oaks’ actual vendors and retention practices before data collection is activated. Cardinal Oaks currently identifies North Carolina as its licensed state.
1. Scope
This Privacy Notice describes how Cardinal Oaks Financial Group LLC (“Cardinal Oaks,” “we,” “us,” or “our”) may collect, use, disclose, retain, and protect personal information through this website and related insurance inquiry services. It does not replace any separate privacy notice required for a particular financial or insurance product.
2. Information we may collect
Depending on the service requested, we may collect identifiers and contact information, such as name, email address, telephone number, ZIP code, state, and communication preferences; inquiry details, such as the type of coverage requested; records of communications and consent; and technical information, such as IP address, browser type, device information, security logs, and essential cookie or session data.
We do not ask you to submit Social Security numbers, Medicare beneficiary identifiers, payment card data, bank information, medical diagnoses, or detailed health information through a general website form. Do not send those items unless a licensed representative directs you to an approved secure channel.
3. How we may use information

- Match an inquiry with an appropriately licensed representative.
- Communicate about a requested service and, only with separate permission where required, send marketing communications.

Website inquiry details are transmitted through our hosting service and Resend, our transactional email-delivery provider, for delivery to Cardinal Oaks. The general inquiry form does not create a customer account or store an inquiry in a website database.
We do not sell personal information for money. Some state laws may define certain advertising or analytics disclosures as “sale,” “sharing,” or “targeted advertising.” If Cardinal Oaks later uses those technologies, this notice and the website’s choice mechanisms must be updated before use.
5. Medicare beneficiary information
Personal beneficiary data collected for Medicare marketing or enrollment will not be shared with another third-party marketing organization unless the individual provides the prior express written consent required by applicable CMS rules. Any such consent must be clear, prominently displayed, specific to each recipient, and maintained as required by law.
6. Retention
We intend to keep personal information only as long as reasonably necessary for the purposes described, to satisfy legal and regulatory recordkeeping duties, resolve disputes, and enforce agreements. Cardinal Oaks must adopt a written retention schedule before activating collection. When information is no longer required, it should be securely deleted or de-identified.
7. Security
We use administrative, technical, and physical safeguards designed to protect personal information. No internet transmission or storage system can be guaranteed completely secure. Do not send sensitive information through ordinary email or an unsecured web form.
8. Your choices and rights
You may opt out of marketing email using the unsubscribe method provided in the message. Depending on where you live, you may also have rights to request access, correction, deletion, portability, or information about certain disclosures, and to appeal a decision. We may verify identity before completing a request and may retain information where permitted or required by law.

9. Children
This website is intended for adults and is not directed to children under 13. We do not knowingly collect personal information from children under 13 through this website.
